When the flying anvil chop a limb the equipment remain in place, placing a new limb retains the equipment.

I've seen this issue a fair number of times before, and I'm pretty sure it's a duplicate of the general phantom item bug, which Kep has claimed to have fixed but it appears to not be fixed I think...
