Is there a standard starting distance between combatants, or is there a random element to it? Any idea if the modifiers that influence golems also influence opponents?

I noticed every critter seems to have a different distance between them and the attacking golem. Some are within 55-60 feet, others at about 100 feet. And the environment seems to affect them as well though breath attacks do not count like normal ranged attacks concerning higher miss rate and some creatures may have terrain penalty immunities as well.

I always thought it started at 100 in round zero, which barring a glitch, you never see. Then round 1 which is the first, sometimes only, closing round that you see in the combat report. The distance between your golem and the target is affected by speed of both with modifiers from terrain.
